TheraCare Home Health is seeking a compassionate and experienced Start of Care Weekend Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. In this role, you will manage a caseload of patients, complete comprehensive assessments, and coordinate care across the interdisciplinary team. You will play a key role in supporting patients throughout their home health episode while promoting independence, safety, and positive clinical outcomes.

Qualifications

Qualifications:

  • Graduate of an accredited Diploma, Associate, or Bachelor's degree in Nursing
  • Current Registered Nurse (RN) license in the state of Texas
  • Minimum 2 years of RN clinical experience; home health experience preferred
  • Strong knowledge of home health regulations, OASIS documentation, and care planning
  • Experience completing Start of Care (SOC), Recertifications, and Resumption of Care visits preferred
  • Ability to manage a patient caseload and work independently in a field-based environment
  • Experience coordinating care with interdisciplinary teams (PT, OT, ST, MSW, HHA)
  • Strong clinical assessment, documentation, and critical thinking skills
  • Excellent communication and interpersonal skills with patients, families, and physicians
  • Reliable transportation with valid driver's license and auto insurance
  • Proficiency with EMR systems (HomeCare HomeBase preferred)
Essential Functions
  • Manage and coordinate all aspects of home health patient care and case management
  • Perform Start of Care (SOC), Recertification, and Resumption of Care assessments
  • Develop, implement, and update individualized plans of care based on patient needs
  • Coordinate care with physicians and interdisciplinary team members to ensure continuity of care
  • Supervise and provide clinical direction to LVNs and Home Health Aides
  • Lead and participate in interdisciplinary case conferences (IDG/IDT)
  • Monitor patient progress and adjust care plans to achieve desired outcomes
  • Provide patient and caregiver education on disease management, medications, and home safety
  • Complete accurate and timely clinical documentation in compliance with regulatory standards
  • Coordinate discharge planning and ensure smooth transitions of care
  • Monitor visit utilization, productivity, and patient outcomes
  • Maintain compliance with agency policies, procedures, and state and federal regulations
