Hospice Registered Nurse (RN) Weekend Warrior
Manageable caseloads, flexibility, and meaningful hospice work that allows you to focus on quality patient care.

Join our team as a Weekend Warrior RN, supporting patient care from Friday through Monday. This full-time position offers flexibility in scheduling. For example, a typical schedule may start around 2:30 PM on Friday and continue through Monday morning (around 10:00 AM), but we are open to adjusting hours to fit the right candidate while maintaining weekend coverage.

Schedule, Hours & Territory:
  • Weekend coverage from Friday through Monday with flexible start and end times to fit your schedule.
  • This role includes on-call responsibilities, along with completing scheduled visits as needed.
  • Territory can span from West Branch to Gaylord, MI.

Key Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized plans of care.
  • Provide skilled nursing care in accordance with hospice standards and physician orders.
  • Coordinate care with physicians, caregivers, and interdisciplinary team members.
  • Educate patients and families on symptom management, disease progression, and end-of-life care.
  • Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase).
  • Participate in interdisciplinary team meetings and care planning.
  • Support after-hours patient needs as part of an on-call rotation when applicable.

Requirements:
  • Active Registered Nurse (RN) license.
  • Hospice or home health experience preferred.
  • Strong clinical assessment and care coordination skills.
  • Ability to work independently in the field.
  • Strong communication and documentation skills.
  • Experience using HCHB (Homecare Homebase) preferred.

Field Role Requirements: 
  • Active driver's license.
  • Ability to pass a criminal background check.
  • Ability to pass a Motor Vehicle Record (MVR) check.
  • Current TB testing in accordance with agency policy.
  • Current CPR certification as required for the role.
