We're looking for a compassionate and skilled Registered Nurse (RN) who thrives in a more personal, patient-focused setting. In this role, you'll have the autonomy to manage your own patients, build meaningful relationships, and deliver high-quality care in the home-whether supporting recovery or providing comfort-focused hospice care.

What You'll Do

  • Provide skilled nursing care, assessments, and treatments in patients' homes
  • Develop and manage individualized care plans for home health and/or hospice patients
  • Monitor patient progress and adjust care as needed
  • Collaborate with physicians, caregivers, and the interdisciplinary team
  • Educate patients and families on care, medications, and disease progression
  • Complete timely and accurate clinical documentation
