Compensation: Guaranteed base, uncapped potential, processed bi-weekly (26 pay periods).

Summary:
We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient's care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.
Responsibilities:
  • Assumes responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.
  • Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
  • Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
  • Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
  • Participate in on-call rotation as reasonably assigned.
Qualifications:
  • Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
  • Active State Registered Nurse licensure is required.
  • Proof of current CPR certification and valid Driver's license.
