Mercy Hospice, Canton OH
Schedule: 3- 12's, every other weekend
Tues- Fri shift would be 12p-12a
Saturday and Sunday shifts will be 8a-8p
Holiday rotation
SUMMARY
The RN After Hours utilizes the nursing process and evidence-based practice to provide patient driven, holistic care with the collaboration of the Interdisciplinary Team, Medical Director, Primary Care Physician or Attending Physician and Expanded Team (family/caregiver and internal and community-based services involved in providing care to the patient). Develops care protocols that support care for patients receiving hospice services throughout the continuum to assure that care is timely, appropriate, of high quality, and cost effective. Uses health education and technology to promote therapeutic relationships and share decision making between the patient and the interdisciplinary team to support patient self-management, prevention and comfort. Demonstrates leadership in the delivery of care through collaborative strategies with others.
The RN After Hours will provide skilled nursing care in a patient's home as directed by the medical director and/or attending physician. The RN After Hours will manage and facilitate resource applications to meet patient needs. If there are minimal to no after-hours needs during the shift, the RN On-Call will support chart review and patient phone calls from their home. They are responsible for all practices and duties within the scope of practice outlined by the state. The RN After Hours is responsible for any and all practices and duties within the state specific scope of practice.
KEY RESPONSIBILITIES

  • Observes and monitors patient conditions and performs comprehensive assessments to develop an individualized care plan and adjusts as needs change.
  • Evaluates the treatments and medical condition of the patient according to the plan of care.
  • Determines if the level of care being provided meets the patients' needs and facilitates level of care changes including the management of after-hours Routine, General Inpatient, Respite and Continuous Care.
  • Administer medication as prescribed by the physician and collaborates on required updates to the plan of care to achieve comfort and symptom control.
  • Prioritizing visits for high-risk patients.
  • Manage multi-disciplinary care as applicable while promoting continuity of care with collaborative determinations on level of care provisions, additional resource need identification, and schedule management.
  • Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
  • Complete chart reviews and answer patient calls, to support patient care and regulatory compliance.
  • Complete assigned workflow timely and with high accuracy.
  • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
  • Other duties as assigned.
QUALIFICATIONS
  • Registered Nurse with current license in the state of employment.
  • 2 years of hospice experience preferred.
  • Previous experience with clinical management, hospice case management, or other relevant clinical experience.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
  • Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.
  • Knowledgeable on nursing best practices.
  • Ability to make appropriate nursing judgments.
  • Ability to identify a situation and handle it with the best possible solution.
  • Detail-oriented and observant.
  • Ability to work autonomously and have efficient time management.
  • Ability to follow directions and work as a team member.
  • Valid driver's license and auto insurance with your name as a listed driver.
  • Designated workspace at home to assist with chart reviews.
