Join a Team That Truly Supports You!

Senior Care Solutions is a Medicare-certified home health agency committed to delivering exceptional, patient-centered care while fully supporting our clinicians in the field. We understand the realities of home health — documentation, scheduling, compliance — and we’ve built strong systems and a collaborative team so you can focus on what you do best: caring for patients.

If you’re an experienced Registered Nurse who values autonomy, flexibility, and a supportive clinical team, and would like to work only weekends, we’d love to meet you.

About the Role

As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes, following physician orders and Medicare guidelines. You’ll work independently in the field while being fully supported by our Clinical Managers, QA team, and office staff.

Key Responsibilities
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits

  • Complete comprehensive assessments and develop individualized plans of care

  • Provide skilled nursing services including medication management, wound care, patient education, and chronic disease management

  • Accurately complete OASIS and clinical documentation in our EMR in a timely manner

  • Collaborate with physicians, therapists, and interdisciplinary team members

  • Educate patients and caregivers to promote safety, independence, and improved outcomes

  • Adhere to all Medicare, ACHC/CHAP, and agency policies and procedures

Qualifications
  • Ability to work weekends

  • Active RN license in the state of Florida (required)

  • Minimum 1 year of Medicare home health experience strongly preferred

  • Experience with Wellsky/Kinnser preferred

  • OASIS experience preferred

  • Current CPR certification

  • Valid driver’s license, reliable transportation, and active auto insurance

  • Strong clinical assessment and time-management skills

  • Comfortable working independently while staying connected to a team

What We Offer
  • Competitive pay per visit or salary options

  • Flexible scheduling and manageable territories

  • Supportive leadership and accessible clinical management

  • Dedicated QA support to help with documentation and compliance

  • Streamlined EMR and strong operational support

  • Opportunities for growth and long-term stability

  • A respectful, team-focused culture that values your time and expertise
