St. Elizabeth Healthcare is hiring a Registered Nurse Case Manager for the Hospice unit located at the Edgewood, Kentucky location.
Shift: 8:00 am - 4:30 pm; Monday - Friday.
Hours: Full-time, 40 hours weekly.

The RN Case Manager is responsible for provision of quality care to all patients and the achievement of excellent patient outcomes. The RN Case Manager assesses patient needs, assists in the formulation and implements the physician's plan of care to reach achievable goals mutually agreeable to the patient and care providers. These activities are carried out in a collaborative manner that promotes quality, continuity and safety. Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background. MINIMUM REQUIREMENTS
Education, Credentials, Licenses:
Graduate of an accredited school of nursing. If eligible, must have a Compact (multi-state) License in their primary state of residence. Licensed to practice nursing in the state where work is being performed. Meets contact hour requirements for licensure, including all state-required courses. Valid Driver's license.
Specialized Knowledge:
Demonstrates advanced assessment skills appropriate for home care environment. Ability to function independently. Knowledge of community resources.
Kind and Length of Experience:
1 year of RN experience in a hospital or care facility setting.
