JOB SUMMARY
Reporting to the RN Manager of Case Management, the RN Case Manager Lead will supervise our team of RN Case Managers in the inpatient and emergency departments at all NCH campuses. The successful candidate will have strong leadership and team-building skills, possess excellent communication skills and problem-solving abilities. The RN Case Manager Lead will act as a resource for the RN Case Managers while ensuring the delivery of high-quality, patient-centered care that focuses on patient experience. The RN Case Manager Lead will also play a key role in program development, staff training, and community outreach.
ESSENTIAL DUTIES AND RESPONSIBILITIES

• Responsible for supporting the Director of Case Management and Social Work Manager in implementing and enforcing NCH and departmental policies and procedures.
• Supervising, orienting, and mentoring of RN Case Managers.
• Reinforces compliance with department documentation standards of initial comprehensive assessments, readmission assessments, progress, and discharge notes.
• Ensures comprehensive individualized discharge planning that meets the needs of patients and their families.
• Identifies gaps in services and develop strategies to improve patient care and resource utilization.
• Participates and promotes quality of care initiatives, length of stay, readmissions, and avoidable days.
• Provides patients/families with resources to meet their medically related social and emotional needs.
• Ensures safe transition of our patients from one care environment to another.
• Promotes patient/family involvement and ensures choice in discharge planning.
• Collaborates with multi-disciplinary teams, healthcare providers and community partners to ensure comprehensive patient care and care transition.
• Possesses in-depth knowledge of case management principles, healthcare practices, and regulatory requirements.
• Stays up to date on evidence based best practices in case management and in collaboration with the leadership team, integrates new approaches into the team's workflow.
• Evaluates program effectiveness and recommends improvements based on data and feedback.
• Provides direct support for complex cases, attending weekly complex case review.
• Promotes and fosters a collaborative, supportive and efficient team culture.
• Provides direct support to complex cases including but not limited to guardianship, unfunded and undocumented patients.
• Collaborates with utilization management team to ensure timely delivery of regulatory forms, appropriate level of care, long stay outliers, and patients exceeding geometric length of stay.
• Works collaboratively with social work partners, the multidisciplinary team and community partners.
• Assists in staffing, as needed.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
• Minimum of Associate Degree in Nursing required; BSN preferred.
• A minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management is required.
• Licensed as a Registered Nurse in the state of Florida.
• Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association
• Case Management Certification Preferred
• Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
