Key Responsibilities
  • Utilize your education, training, and experience to formulate independent psychosocial diagnoses for our patients, tailoring approaches to their individual needs.
  • Collaborate with patients to create effective, patient-centered treatment plans that align with their personal goals and care preferences.
  • Implement evidence-based behavioral interventions, including solution-focused therapy, motivational interviewing, and other appropriate methodologies, to promote mental well-being.
  • Engage in multi-disciplinary coordination with other healthcare professionals, including physicians and nursing staff, to ensure a holistic approach to patient care.
  • Systematically monitor treatment progress, documenting outcomes, and adjusting treatment plans as necessary to enhance patient efficacy.
  • Assist with psychotropic medication management in line with healthcare provider recommendations, providing oversight on adherence, side effects, and overall treatment effectiveness.
Patient Population / Client Focus

The focus of this role will be on patients receiving hospice care, which often involves addressing complex emotional, social, and psychological challenges associated with terminal illnesses. You will work closely with patients and their families, navigating grief, providing crisis intervention, and fostering coping strategies to enhance their quality of life during this critical time.

Qualifications
  • A Master’s degree in Social Work from an accredited program is required.
  • Must possess a current, valid, and unrestricted license to practice independently in mental health as a Licensed Clinical Social Worker (LCSW) in the state of Maryland.
  • At least 1 year of experience as an independently licensed therapist, particularly in settings involving end-of-life care or hospice services is preferred.
Preferred Experience

Experience working with hospice or palliative care patients is strongly preferred, as well as familiarity with issues surrounding grief, loss, and bereavement counseling. Knowledge of community resources and support networks in Silver Spring and the broader Maryland area can enhance your effectiveness in this role.
