• Conduct thorough psychosocial assessments through detailed patient interviews, observations, and the review of medical records to evaluate factors impacting recovery.

• Work collaboratively with physical and occupational therapists, physicians, and other rehabilitation professionals to develop individualized treatment plans that incorporate the patients' psychosocial needs.
• Guide patients in accessing community resources, including support groups, vocational rehabilitation programs, and housing assistance, facilitating their reintegration into the community.
• Educate patients and families on available resources for post-rehabilitation services, empowering them to navigate their journeys independently.
• Participate actively in discharge planning to ensure smooth transitions for patients moving back home or to other care settings.

You will work with a diverse patient population, each facing unique challenges related to recovery and adjustment after medical incidents, such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. By addressing the emotional and social factors that influence rehabilitation, you will significantly contribute to helping these individuals regain their independence and re-engage with their communities.

• A Master's degree in Social Work from an accredited institution is mandatory.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Strong background in clinical social work practices, particularly in a healthcare or rehabilitation setting.
• Excellent communication and interpersonal skills are essential for building rapport and effectively supporting patients and their families.
• A solid understanding of coping mechanisms, grief, and adjustment issues associated with rehabilitation.
