Work Environment

This position is designed for a licensed Clinical Social Worker who will work out of our Hospice Center, providing remote support to clients. Key Responsibilities
  • Conduct thorough psychosocial assessments to understand clients' mental health concerns and establish support mechanisms.
  • Provide individual, family, and group therapy sessions utilizing evidence-based modalities such as cognitive-behavioral therapy, dialectical behavior therapy, and solution-focused therapy.
  • Design and implement individualized treatment plans that include measurable goals and tailored interventions.
  • Collaborate closely with a multidisciplinary treatment team to ensure a seamless continuum of care for each client.
  • Facilitate psychoeducation classes and support groups to help clients develop coping strategies and resilience.

Patient Population / Client Focus

Qualifications
