The Cognitive Care Center is seeking a motivated and fully licensed Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity, with a special focus on improving the lives of seniors. As a remote position, you will enjoy the flexibility of working from home while still being integral to our school-based programs. Your schedule will primarily involve regular office hours, Monday to Friday from 8:00 AM to 4:30 PM, with some evenings or weekends required based on client needs. As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments of seniors to identify their emotional, mental, and social needs.
• Developing personalized care plans in collaboration with seniors, their families, and interdisciplinary teams to enhance care delivery.
• Providing individual and family counseling sessions aimed at helping clients manage life transitions and resolve conflicts.
• Advocating for clients to ensure they have access to necessary community resources, support services, and rights within the healthcare system.
• Engaging in regular follow-ups to monitor clients' conditions and adjust care plans as needed, utilizing electronic health record (EHR) systems for documentation.
• Educating clients and families on community resources, support groups, and programs designed for seniors to foster social engagement.
• Collaborating closely with nursing staff, healthcare professionals, and community partners to ensure holistic care.
• Staying current with social work practices, policies, and ethical standards to maintain professional integrity.

Your work will primarily focus on seniors, who often face an array of challenges, including mental health issues, social isolation, and significant life transitions. By understanding their unique needs and circumstances, you'll play an essential role in promoting their dignity and independence. Your holistic approach will ensure that they receive not only the necessary care but also encouragement and emotional support throughout their aging journey.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of relevant experience in a social work role, preferably focused on elderly clientele or within a community-based setting.
• Strong understanding of aging-related issues and familiarity with available social resources within the Kansas City community.
• Excellent communication and problem-solving abilities, with a compassionate approach to client interactions.
• Proficient in using electronic health records (EHR) and standard software applications.

While not mandatory, experience working within school settings or with interdisciplinary care teams would be viewed favorably. Familiarity with various community programs and resources that specifically address the needs of seniors will enhance your effectiveness in this role.
