Position Overview

The Cognitive Care Center is seeking a motivated and fully licensed Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity, with a special focus on improving the lives of seniors. You will embody the center's mission to provide effective, compassionate, and comprehensive support to the elderly population and their families, helping them navigate challenges and ensure their overall well-being.

About the Organization

The Cognitive Care Center is dedicated to enhancing the quality of life for seniors in our community through personalized interventions and compassionate care. We understand the complexities that come with aging and the diverse needs of seniors as well as their families. Our organization prides itself on a multidisciplinary approach, working collaboratively with healthcare providers, caregivers, and community resources to deliver exceptional services tailored to each individual's unique circumstances.

Work Environment

As a remote position, you will enjoy the flexibility of working from home while still being integral to our school-based programs. Your schedule will primarily involve regular office hours, Monday to Friday from 8:00 AM to 4:30 PM, with some evenings or weekends required based on client needs. Being part of our dynamic team means you will engage with clients and their families via virtual platforms, fostering strong relationships despite geographic distances.

Key Responsibilities

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

  • Conducting comprehensive psychosocial assessments of seniors to identify their emotional, mental, and social needs.
  • Developing personalized care plans in collaboration with seniors, their families, and interdisciplinary teams to enhance care delivery.
  • Providing individual and family counseling sessions aimed at helping clients manage life transitions and resolve conflicts.
  • Advocating for clients to ensure they have access to necessary community resources, support services, and rights within the healthcare system.
  • Engaging in regular follow-ups to monitor clients' conditions and adjust care plans as needed, utilizing electronic health record (EHR) systems for documentation.
  • Educating clients and families on community resources, support groups, and programs designed for seniors to foster social engagement.
  • Collaborating closely with nursing staff, healthcare professionals, and community partners to ensure holistic care.
  • Staying current with social work practices, policies, and ethical standards to maintain professional integrity.
Patient Population / Client Focus

Your work will primarily focus on seniors, who often face an array of challenges, including mental health issues, social isolation, and significant life transitions. By understanding their unique needs and circumstances, you'll play an essential role in promoting their dignity and independence. Your holistic approach will ensure that they receive not only the necessary care but also encouragement and emotional support throughout their aging journey.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of two years of relevant experience in a social work role, preferably focused on elderly clientele or within a community-based setting.
  • Strong understanding of aging-related issues and familiarity with available social resources within the Kansas City community.
  • Excellent communication and problem-solving abilities, with a compassionate approach to client interactions.
  • Proficient in using electronic health records (EHR) and standard software applications.
Preferred Experience

While not mandatory, experience working within school settings or with interdisciplinary care teams would be viewed favorably. Familiarity with various community programs and resources that specifically address the needs of seniors will enhance your effectiveness in this role.

Compensation and Benefits

We offer a competitive salary ranging from USD 60,000 to 100,000 annually, depending on experience and expertise. In addition, our comprehensive benefits package includes:

  • Medical, dental, and vision insurance to support your health and well-being.
  • A retirement savings plan with employer-matching contributions to help secure your financial future.
  • Generous paid time off (PTO) to promote work-life balance and avoid burnout.
  • Professional liability insurance to protect you in your practice.
  • Opportunities for continuing education and support for your professional development.
Career Growth Opportunities

At Cognitive Care Center, we believe in investing in our employees' growth and career paths. As a Licensed Clinical Social Worker, you will have access to ongoing training and mentorship, facilitating skills enhancement and career advancement within our organization and the broader community. With dedication and performance, you may explore leadership positions or specialized roles catering to your interests.
