Job Summary

The licensed clinician provides psychosocial evaluations, psychotherapeutic interventions, case management and discharge planning. Focuses upon transitions in care for assigned patient populations. Provides screening, assessment, planning, problem resolution, resource management, social work services, therapy and crisis management as they relate to behavioral health care and discharge planning needs of the patient/family. Provides and acts as a liaison to facilitate a comprehensive and individualized plan of care among physician, patient and/or family support systems, community agencies and facilities, and other entities within patient system of care.

Essential Functions

  • Develops individualized treatment plans which encompass discharge planning and follow up, including but notlimited to facilitation of alternative placement, coordination of medical equipment, and case collaboration with referral resources to support transition/discharge to the next level of care.

  • Prepares case history of patient's personal, social, and emotional development.

  • Assists patients in the evaluation of their strengths and symptoms and the incorporation of these into personalized treatment goals

  • Provides therapeutic intervention with patient\family including individual, marital, parental, family, and grouptherapies.

  • Provides education related to patient's diagnosis

  • Communicates patient and family issues to treatment team and acts as the liaison between other active services and/or services involved in patient's discharge plan

  • Assists patients in locating and utilizing community resources including legal, financial assistance, and otherreferral services.

  • Strives to expand the variations of service they provide by integrating community-based programs to provide home and community-based treatment options for the identified patient/family.

  • Responsible for clinical updates and advocates with 3rd party payers when required.

  • Documents all social service interventions and maintains patient and program records in accordance withapplicable standards and regulations.

  • Monitors caseload and quality review to assure compliance for third-party payers and regulatory agenciesincluding, but not limited to, clinical documentation, chart review, completion of prior request andauthorizations.

  • Assumes responsibilities for making sure individualized treatment plan is implemented with patient/family

  • Demonstrates a working knowledge of psychological, behavioral, physical, health, and medical sequela of various disabilities and incorporates this knowledge into treatment planning.

  • Demonstrates a working knowledge of various supportive, vocational, and community resources addressing and supporting recovery of various psychological, behavioral, and physical disabilities.

  • Demonstrates knowledge of age-appropriate interventions and cultural differences as applied to the recoveryprocess.

  • Coordinates outpatient scheduling including initializing the recertification of services.

  • Applies facets of Mental Health First Aid during times of crisis to patients.

  • Enhances professional growth and development through professional affiliations, participation in educational programs, in-service meetings, workshops and review of current literature.

  • Participates in planning conferences and peer supervision, adhering to a professional code of ethics.

  • Identifies and works with potential community support systems such as neighborhood networks and communityorganizations to support patient transition and discharge planning.

  • Performs as Designated Examiners which involves the commitment process, designated exam, and testimony for probate courts.

  • Provides oversight and supervision to students from local universities. Assists student interns from affiliated schools in the professional development of clinical skills.

  • Collaborates with the department's leadership team in assessing department needs and designing systems to meet department and organizational strategic initiatives.

  • Coordinates with utilization review to determine need for authorization of payer of active or post-acute services.

  • Maintains program statistics, through internal audits for purposes of evaluation and research.

  • Participates in weekend on call rotation.

  • Provides intake and psychosocial assessments for all patients in accordance with hospital and departmental policy and procedures/regulation requirements.

  • Assesses for immediate discharge planning needs, develops and coordinates a discharge plan as determined by the patient's mental status, level of independence, functional capacity, family/social support systems, and financial resources.

  • Assesses preadmit resources and services as well as support systems

  • Determines appropriate follow up and referral services as identified by the interdisciplinary team.

  • Participates in following and monitoring corporate compliance standards.

  • Participates in program evaluation, CQI studies and performance improvement measures.

  • Provides input into formulation or revision of departmental policies and procedures.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Master's degree inSocial Work or Counseling Psychology or related field of study.

  • Experience - One (1) year inpatient psychiatric hospital experience and/or clinical assessments and case management experience. Behavioral health/counseling psychology experience preferred.
