What You'll Do

  • Complete routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities.
  • Assess changes in condition and provide expert pain and symptom management.
  • Deliver wound care, medication education, and comfort-focused nursing interventions.
  • Support patients and families through compassionate communication and education.
  • Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
  • Complete timely, accurate EMR documentation.

What You'll Bring

  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, or end-of-life care.
