As a PRN Hospice RN, you will provide skilled nursing care, perform patient assessments, manage pain and symptoms, educate patients and caregivers, and collaborate with our interdisciplinary team to ensure every patient receives compassionate, individualized care focused on comfort, dignity, and quality of life.
Whether you're providing routine visits, admissions, or assisting with patient coverage during fluctuations in census, you'll play a vital role in supporting our patients, families, and care team.
Primary Responsibilities
  • Perform initial, comprehensive, and ongoing nursing assessments of hospice patients and their caregivers.
  • Provide skilled nursing care in accordance with the individualized plan of care and hospice standards of practice.
  • Assess, monitor, and manage pain and symptoms while promoting patient comfort and quality of life.
  • Collaborate with physicians and the interdisciplinary team to develop, implement, and update individualized plans of care.
  • Communicate changes in patient condition and obtain physician orders as appropriate.
  • Educate patients and caregivers on disease progression, medications, symptom management, safety, and the end-of-life process.
  • Complete timely and accurate clinical documentation within the electronic medical record (EMR).
  • Provide compassionate emotional support to patients and families while maintaining the highest standards of hospice care.
  • Support patient care needs by assisting with routine visits, admissions, and coverage based on patient census and operational needs.
  • Demonstrate professionalism while supporting the mission, vision, and values of the organization.

Qualifications
Education
  • Graduate of an accredited school of nursing required.
  • Bachelor of Science in Nursing (BSN) preferred.
Licensure & Certifications
  • Current, unrestricted Registered Nurse (RN) license in the State of North Carolina.
  • Current American Heart Association (AHA) Basic Life Support (BLS/CPR) certification.
  • Valid North Carolina driver's license.
  • Reliable transportation with current automobile insurance that meets state requirements.
Experience
Required
  • Minimum of one (1) year of experience as a Registered Nurse.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently while providing compassionate, patient-centered care.

Preferred
  • Previous hospice, home health, palliative care, oncology, ICU, medical-surgical, or other relevant nursing experience.
  • Experience providing end-of-life care and patient/family education.
  • Experience with pain and symptom management.
  • Experience using Homecare Homebase (HCHB) or other electronic medical record (EMR) systems.

Knowledge, Skills & Abilities
  • Strong understanding of hospice philosophy and palliative care principles.
  • Excellent assessment, clinical decision-making, and communication skills.
  • Ability to establish trusting relationships with patients, families, physicians, and interdisciplinary team members.
  • Compassionate, empathetic, and emotionally resilient while caring for patients with life-limiting illnesses.
  • Comfortable using tablets, smartphones, Microsoft Outlook, Teams, and electronic medical records.
  • Flexible and adaptable to changing patient needs, schedules, and service areas.
  • Strong organizational and time management skills with the ability to prioritize patient care effectively.

Physical Requirements
  • Ability to stand, walk, bend, lift, and assist patients as needed throughout the workday.
  • Ability to travel throughout the assigned service area to provide in-home patient care.
  • Visual acuity and observation skills necessary to perform comprehensive patient assessments.

Work Environment
  • Frequent travel to patient homes and other care settings within the assigned territory.
  • Exposure to infectious diseases and healthcare-related hazards consistent with patient care.
  • Work performed in a variety of home and community environments.
