Job description

CareSense is seeking a Pediatric Registered Nurse (RN) to support our growing home care program. This is a hybrid position, with time split between the office (documentation, coordination, and case management) and the field (supervisory home visits). The RN will play a key role in ensuring our pediatric patients receive safe, high-quality care in their homes.
Key Responsibilities
  • Travel to pediatric patients' homes to complete RN supervisory visits in accordance with payer and agency requirements.
  • Complete and update CMS-485/Plans of Care and other required clinical documentation for pediatric home care cases.
  • Perform focused nursing assessments and document skilled needs, progress toward goals, and any changes in condition.
  • Review charts in the office to ensure documentation is complete, accurate, and compliant with regulations and payer standards.
  • Collaborate with coordinators, administrators, and the clinical team to resolve issues, support staffing, and ensure continuity of care.
  • Communicate with physicians, case managers, and other providers regarding patient status, care needs, and plan-of-care changes.
  • Provide education and support to caregivers and families on the child's care needs, equipment, and safety.
  • Offer clinical guidance to direct care workers/home care aides working with pediatric clients.

Qualifications
  • Active Registered Nurse (RN) license in good standing in the state of Pennsylvania.
  • Pediatric experience and/or home care experience preferred (hospital, clinic, or home health/hospice all a plus).
  • Valid driver's license, reliable transportation, and willingness to travel to patients' homes within the service area.
  • Strong documentation skills and comfort using EMR/electronic documentation systems.
  • Ability to work independently in the field while also functioning as part of a team in the office.
  • Excellent communication, organization, and time-management skills.
