Nurse Practitioner โ€“ Nursing Home / House Call (Outpatient, Part-Time Options Available)

Position Type: Full-Time or Part-Time (Flexible, including weekends only) Work Environment: Nursing Home / House Call (Outpatient) Location: Manhattan, NY (House Calls needed in Bronx, Nursing Home care in Manhattan)

Job Summary:

A large, well-established multispecialty group with locations throughout the Greater NYC area is seeking Nurse Practitioners to provide care in either nursing homes or through house calls. This is a permanent opportunity with flexible scheduling, strong clinical support, and the option to select the borough that works best for you.

For house calls, a car service will be provided. Spanish language skills are preferred but not required.

Essential Job Functions:
  • Provide comprehensive medical care in nursing home or house call settings

  • Evaluate, diagnose, and manage acute and chronic medical conditions

  • Order and interpret diagnostic tests, prescribe medications, and create treatment plans

  • Educate patients and caregivers on preventive care, treatment, and follow-up plans

  • Collaborate with physicians, APPs, and support staff to deliver high-quality care

  • Accurately document patient encounters in the EMR
