Job description

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Admissions Registered Nurse (RN) to support patients and families throughout Palm Beach County. This role offers a consistent weekend-only schedule—every Friday, Saturday, and Sunday from 9:00am to 9:30pm, making it an excellent opportunity for nurses seeking a three-day workweek, work–life balance during the week, or an alternative to traditional weekday schedules.

What you'll Do
As an Admissions Nurse, you will play a vital role as both a clinical expert and a trusted guide for patients, families, and healthcare partners:

  • Serve as the primary nurse responsible for evaluating patients referred for hospice services
  • Educate patients and families on hospice care and clearly communicate all components of our continuum of care
  • Collaborate with referring physicians and liaisons to coordinate individualized, hospice-appropriate plans of care
  • Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long-term care facilities, and other healthcare professionals
  • Promote and advocate for the hospice philosophy through education and relationship-building within the healthcare community

What You'll Need
  • Registered Nurse currently licensed in the State of Florida.
  • Current Basic Life Support Certification.
  • Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
  • Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
  • Basic computer skills and EMR software experience a plus.
  • Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
