Job description

The PRN On-Call Hospice RN provides after-hours nursing support to hospice patients and families, ensuring timely response to urgent needs, symptom management, and coordination of care consistent with the hospice plan of care.

Employment Status

  • PRN / As Needed

  • Non-exempt or exempt based on agency structure

  • On-call shifts (evenings, nights, weekends, holidays)

Primary Responsibilities

  • Responds to patient/family calls during assigned on-call hours

  • Provides telephonic triage and clinical guidance

  • Determines need for and performs urgent visits

  • Assesses patient condition and changes in status

  • Implements interventions for pain and symptom control

  • Administers medications and treatments per hospice plan of care

  • Provides end-of-life care and support

  • Manages acute symptom crises (e.g., pain, dyspnea, agitation)

  • Provides education and reassurance to families during end-of-life transitions

  • Coordinates with physician or nurse practitioner as needed

  • Communicates with hospice physician/NP regarding changes in condition

  • Updates plan of care as appropriate

  • Coordinates with pharmacies, DME providers, and other vendors

  • Completes timely, accurate documentation of all calls and visits

  • Documents assessments, interventions, patient response, and follow-up plan

  • Educates on symptom management, medication use, and what to expect

  • Reinforces hospice philosophy and goals of care

Qualifications

Required

  • Current, unencumbered Registered Nurse (RN) license - Wyoming

  • Minimum 1–2 years nursing experience (hospice, home health, or acute care preferred)

  • Valid driver’s license and reliable transportation

  • Ability to work independently and make clinical decisions

Scheduling Expectations

  • Assigned on-call shifts (e.g., evenings 5pm–8am, weekends, holidays)

  • Minimum of one weekend per month

  • Timely response to calls and visits
