Description:OB Nurse Case Manager (Remote – Indiana Residents Only)


Location: Remote (must reside in Indiana)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM ET
Pay: $90,000–$93,000 annually (based on experience)
Benefits: Health, Dental, Vision, 401(k), 10 Paid Holidays, and 80 Hours PTO


About the Role

We are seeking a dedicated and experienced OB Nurse Case Manager to join our team supporting expectant mothers throughout their pregnancy and postpartum journey. This role requires a minimum of three (3) years of Labor & Delivery experience, as you’ll serve as a trusted guide and educator for members navigating pregnancy, delivery, and recovery.

You’ll play a key role in improving maternal outcomes by coordinating care, providing education, and connecting members with the services and resources they need. This is a fast-paced, full-time position that requires strong organization, critical thinking, and compassion.


What You’ll Do
  • Manage a caseload of pregnant members, providing telephonic support and care coordination
  • Educate and guide members through pregnancy, delivery, and postpartum care.
  • Conduct comprehensive assessments to identify individual needs and develop care plans.
  • Coordinate medical appointments, referrals, and follow-up care across providers and community resources.
  • Monitor care plans, evaluate progress, and make adjustments as needed.
  • Ensure members have access to appropriate services based on their health needs.
  • Maintain accurate documentation in insurance systems and adhere to compliance standards.
  • Collaborate with physicians, social workers, and other care team members to optimize member outcomes.
Requirements:What We’re Looking For
  • Education: Bachelor’s degree in a health-related field preferred.
  • Experience: Minimum of 3 years of clinical Labor & Delivery experience (required).
  • Licensure: Active, unrestricted RN license in Indiana (multi-state licensure preferred).
  • Certification: Case Management certification preferred.
  • Strong organization, attention to detail, and time management skills.
  • Compassionate communicator with a passion for supporting expectant mothers.
  • Must be able to commit to a full workday — this is a busy, fast-paced role that requires focus and dedication.
