Assigned Territory
This position serves patients throughout the St. Paul, MN area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in St. Paul, St. Louis Park, Roseville MN, and surrounding communities. Daily travel within the assigned territory is expected to support patient care and operational needs.
Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.
What You'll Do
As a Nurse Practitioner with Alteas Health, you'll provide specialty Pulmonology services while working collaboratively with physicians, facility staff, and interdisciplinary care teams to deliver high-quality, patient-centered care.
Responsibilities include:
  • Evaluate, diagnose, and manage acute, chronic, and episodic pulmonary conditions.
  • Develop individualized treatment plans using evidence-based clinical practice.
  • Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
  • Prescribe medications and treatments within your scope of practice.
  • Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
  • Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
  • Build strong relationships with facility leadership and clinical teams.
  • Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.

What We're Looking For
Required Qualifications
  • Current, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
  • Current national certification appropriate to licensure.
  • Current Basic Life Support (BLS) certification.
  • Minimum six (6) months of Nurse Practitioner clinical experience.
  • Valid driver's license with reliable transportation.
  • Ability to travel daily throughout an assigned territory.
  • Strong clinical judgment, communication, organizational, and critical thinking skills.
  • Experience utilizing Electronic Medical Records (EMR).

Preferred Qualifications
  • Two (2) or more years of Nurse Practitioner experience.
  • Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
  • Experience caring for medically complex patient populations.
