Summary:
The Medication Adherence Navigator is responsible for outreaching to and engaging with patients around adhering to medications. The Navigator supports patient experience and clinical outcomes by ensuring patients are able to fill, refill, and take medications as prescribed by their provider. The Medication Adherence Navigator will help to identify barriers to adhering to medications and assist patients with navigating and overcoming challenges related to adherence. The Medication Navigator is a key contributor to the Quality Team by supporting strong performance against priority quality and patient experience measures.
This individual will assess, identify, and prioritize individual needs and builds rapport and trust. In addition, the Navigator will review and assess the member's available data, including clinical/claims history from both outpatient and inpatient treatments to include pharmacy history or other information, to assist in the monitoring and facilitation of adherence to prescribed care plans. The Navigator will collaborate regarding opportunities for optimizing care. This individual will facilitate member understanding of the physician's treatment plan, including but not limited to, prescriptions, refills, medical supplies, referrals, authorization of services, and when to seek care, as well as interview the member and/or family to further assess social, emotional, functional, and physical health status.
Requirements:
Three years clinical experience required. Excellent computer skills, communication, problem-solving, critical-thinking, decision-making, time management, organizational and customer service/service recovery skills. Knowledgeable of patient care operational processes. Demonstrated ability to work autonomously and collaboratively.

Valid and active Pharmacy Technician registration in appropriate state
