Overview
A rapidly growing value-based primary care organization is seeking a Family Medicine Physician to join an outpatient clinic in Des Moines, Iowa.
This practice model focuses on delivering high-quality care for Medicare-aged patients through smaller patient panels, longer visit times, and strong clinical support teams.
The care model is designed to prioritize quality outcomes and preventative care rather than patient volume.

Schedule
Monday – Friday
8:00 AM – 5:00 PM
Full-time position
40 hours per week
Minimal call (shared statewide rotation)

Practice Model
Outpatient primary care clinic
Value-based care environment focused on:
• Preventative medicine
• Chronic disease management
• Population health outcomes
Additional highlights:
• Patient panel capped at ~450 patients per provider
30-minute patient visits

Clinical Support
Each provider works within a collaborative care team including:
• Dedicated Medical Assistant
• Dedicated Medical Scribe
• Shared RN support (1 RN per 2–3 providers)
This team-based model significantly reduces documentation burden and administrative workload.

Responsibilities
• Provide comprehensive primary care services for adult patients
• Diagnose and manage acute and chronic medical conditions
• Conduct preventative care visits and health screenings
• Develop treatment plans and coordinate follow-up care
• Collaborate with interdisciplinary care teams
• Document patient encounters using Epic EMR

Requirements
MD or DO
• Board Certified or Board Eligible in Family Medicine
• Active Iowa medical license
• Active DEA license
Preferred qualifications:
• Experience in primary care or outpatient medicine
• Experience working with Medicare or geriatric populations
• Familiarity with value-based care models
