MDS Enrollment Registered Nurse (RN)

Full-Time | Monday-Friday | 8:00 AM-4:00 PM | No Nights, Weekends, or Holidays

Join a team dedicated to helping older adults access quality community-based healthcare.

We are seeking a compassionate Registered Nurse (RN) to serve as an MDS Enrollment RN. In this role, you'll complete in-home clinical assessments for prospective participants and collaborate with an interdisciplinary team to support the enrollment process. If you enjoy assessment-based nursing, autonomy in the field, and a predictable weekday schedule, we'd love to hear from you.

What You'll Do
  • Conduct in-home assessments and complete the Minimum Data Set–Home Care (MDS-HC)
  • Evaluate medical, functional, psychosocial, caregiver, and home safety needs
  • Present assessment findings to the interdisciplinary team
  • Coordinate additional clinical evaluations as needed
  • Serve as a clinical liaison between prospective participants, caregivers, referral sources, and the care team
  • Maintain accurate electronic documentation and support a seamless enrollment process
Qualifications
  • Active Massachusetts Registered Nurse (RN) license
  • Associate's or Bachelor's degree in Nursing, or completion of an accredited Nursing Diploma Program
  • At least 1 year of nursing experience in geriatrics, home health, long-term care, case management, community health, or adult behavioral health preferred
  • Strong assessment, communication, and organizational skills
  • Experience with electronic medical records and Microsoft Office
  • Valid driver's license and reliable transportation for frequent local travel
Schedule
  • Full-time
  • Monday-Friday
  • 8:00 AM-4:00 PM
  • No nights, weekends, or holidays
  • This is a field-based position with frequent local travel
