Job description

Weekend Float RN Case Manager

Full-Time Hospice Registered Nurse Weekend Coverage | Field-Based Clinical Role

Schedule: Saturday, Sunday and Monday 7:00am-7:30pm; one major and one minor or three minor holidays per year

This position is primarily conducted in the field, providing care to patients wherever they call home, including private residences, assisted living communities, skilled nursing facilities, and other care settings throughout our service area.

Unlike a traditional hospital or clinic-based nursing position, this role requires daily travel between patient locations, independent clinical decision-making, and the ability to build meaningful relationships with patients and families in their own environments.

If you thrive working independently, enjoy being on the move throughout the day, and value one-on-one patient care, this role offers the opportunity to practice nursing in its most personal and impactful setting.

Bring Your Expertise Where It's Needed Most

Some nurses thrive on variety. Others excel when faced with complex clinical situations that require confidence, adaptability, and leadership.

As a Weekend Float RN Case Manager, you'll have the opportunity to do both.

At Transitions LifeCare, we believe every individual deserves the quality of life they want when facing a serious illness or near life's end. Our mission is to care for individuals and families with compassion, support, and expertise as they navigate life-changing illness and end-of-life journeys. This role is critical to ensuring patients receive exceptional hospice care whenever and wherever they need it.

We're seeking an experienced Registered Nurse who can confidently provide field visits, support admissions, manage urgent patient needs, and step into the Case Manager role when census demands or staffing coverage requires it.

If you're a highly skilled clinician who enjoys autonomy, variety, and making a meaningful impact, we invite you to join our team.

Why Join Transitions LifeCare?

As one of North Carolina's most respected nonprofit hospice and palliative care organizations, we're committed to supporting our team members both personally and professionally.
