Job description

 Resident Care Coordinator 

Position Summary: 

Primary responsibility is to oversee the status and health condition of all the residents, coordinate care with family, outside agencies and residents' primary care doctor. You will assist residents regarding health-related issues and delegate caregivers and instruct them  on their daily functions with residents. You will always ensure staff adherence to laws and  regulation. Standards of care will be met and exceed guideline as well as supervising staff.  

Qualifications: 

Caregiver in Assisted Living/Memory Care,  no less than two years experience, RN delegated med Tech with no less than 1-2 years experiene. 

Duties and Responsibilities:  

  • Knowledge of the Enriched Senior Living mission, staff and operations including commitment to "person-directed" care
  •  As the Resident Coordinator, to assist in the preparation and final approval on all service plans and assessments for all residents. Assessments to be completed upon admission, quarterly and change of condition and reviewed with/by Executive Director
  • To act as a liaison for health-related issues to volunteers, families, or the public
  • Attend and participate in monthly care conferences and staff meetings
  • To train and monitor all staff in the handling and dispersing of medications
  • To ensure that all MARs are accurate and kept up to date in BlueStep
  • Coordinate delegation of caregivers with RN delegator
  • LOCA assessments with the final review of Executive Director
  • This job will require, at times, the employee to lift more than 50 pounds helping residents.
  • Proper lifting techniques must be used to avoid injury to staff or residents.
