You will utilize your clinical skills in providing end of life care as directed by an attending physician. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state.**MAJOR AREAS OF RESPONSIBILITY:** Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and make adjustments as needs change. Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs.
