The Hospice RN OC Team in Franciscan Health is responsible for providing total comprehensive nursing to patients and their families through the development of a plan of care. The RN develops these patient-specific plans by performing psychosocial/physiologic assessments, considering patients' cultural/ethical/spiritual needs. The Hospice RN integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum of care. Hospice care may be provided in the home, long term care, assisted living, or inpatient setting.

WHAT YOU CAN EXPECT

  • SIGN ON BONUS AVAILABLE- (Ask your Talent Acquisition Advisor if you are eligible for Sign-On Bonus)

  • Saturday & Sunday- 12 Hour Day Shifts- doing Hospice Admissions.

  • Start and end time flexible- Dependent on patient's needs.

  • Determine patient eligibility for home care and hospice care.

  • Practice nursing, and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.

  • Assess, collect, and analyze comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition, to determine the plan of care.

  • Communicate timely changes in patient's assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care.

  • Promote patient's independence by establishing patient care goals; teach patient, friends, and family to understand the condition, medications, and self-care skills; answer questions.

QUALIFICATIONS

  • Associate's Degree- Nursing - Required

  • Registered Nurse (RN)- State Licensing Board - Required

  • Basic Life Support Program (BLS) - American Heart Association - Required

  • 1 year Home Health or Hospice Care Experience - Required

  • 2 years Long Term Care Experience- Required
