Weekend Baylor Registered Nurse (RN) - Home Health

Our Weekend Baylor RNs provide one-on-one skilled nursing care in the home, helping patients maintain independence while receiving high-quality, personalized care. This role offers autonomy, strong team support, and meaningful patient relationships.
Schedule (Full-Time, Exempt)

  • Saturday & Sunday: 7:00 AM - 8:00 PM
  • Monday: 4 hours for follow-ups and documentation
Key Responsibilities
  • Perform patient assessments and develop individualized care plans
  • Deliver skilled nursing care, including medication and treatment management
  • Monitor patient progress and adjust care as needed
  • Educate patients and caregivers on health management and self-care
  • Coordinate with physicians and interdisciplinary teams
  • Supervise Home Health Aides and LPNs
  • Complete timely and accurate clinical documentation (including OASIS)
Qualifications
  • Active RN license
  • BSN + 1 year experience OR ASN/Diploma + 2 years experience
  • Current CPR certification
  • Valid driver's license and reliable transportation
Preferred
  • Home health experience
  • Behavioral or mental health experience
Physical Demands & Work Environment
  • Frequent standing, walking, sitting, and use of hands
  • Regular bending, kneeling, and reaching
  • Occasional lifting and repositioning of patients
  • Ability to lift/push up to and over 25 lbs as needed
  • Exposure to clinical environments and infectious materials
