PRIDE Health is seeking a local contract Home Health Physical Therapist for a local contract job in Monticello, Kentucky.

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

Compensation:

Local Pay: $60.00 - $70.00/hour (W-2)

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes

Location: Monticello, KY, 42633

  • Schedule details (hours per day/week): Monday- Friday 8:00am eastern to 4:30pm eastern 40 hours/week 8 hours/day
  • Productivity expectations:30 points
  • On Call/Weekend Requirement: no
    Coverage Areas: Wayne County primary may ask to pick up visits in nearby counties if low
    Shift schedule (days, start & end times): Monday- Friday 8:00am eastern to 4:30pm

Key Responsibilities:

  • Evaluate patients to assess physical limitations, strength, balance, mobility, pain levels, and functional abilities.
  • Develop individualized treatment plans based on physician orders and patient needs.
  • Provide therapeutic interventions, including:
  • Exercise programs
  • Gait and mobility training
  • Balance and fall-prevention training
  • Strengthening and endurance activities
  • Pain management techniques
  • Teach patients and caregivers safe movement techniques, home exercise programs, and strategies to improve daily functioning
  • Assess home safety and recommend adaptive equipment or environmental modifications to reduce fall risk
  • Monitor patient progress and modify treatment plans as needed to achieve goals.
  • Document care and outcomes in accordance with agency, state, Medicare, and professional standards.
  • Coordinate care with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare team members
  • Educate patients on disease management and recovery following surgery, injury, illness, or hospitalization.
  • Ensure compliance with regulatory requirements, quality standards, and agency policies.
