• Provide counseling services to patients and their families, helping them navigate the emotional challenges of illness and loss, thus empowering them through this sensitive journey.
• Assist patients in accessing necessary community resources, including support groups, financial aid, and other vital services, ensuring a comprehensive support network.
• Engage in discharge planning, coordinating care transitions from hospice services to home or alternative care settings to facilitate continuity of care.
• Participate in ongoing professional development and training to stay updated on best practices and innovative approaches in social work and hospice care.

The hospice center caters to a broad and diverse patient population, primarily consisting of individuals with terminal illnesses and their families. • A Master’s degree in Social Work from an accredited program is required.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee.
• Strong understanding of grief, coping mechanisms, and adjustment issues faced by patients and families dealing with terminal illnesses.
• Excellent communication, interpersonal, and problem-solving skills are vital for fostering relationships with patients and their families.

While not mandatory, previous experience in a hospice, rehabilitation, or medical setting is preferred. Familiarity with interdisciplinary healthcare teams and an understanding of end-of-life care practices will enhance your capacity to connect with our patient population.
