JOB SUMMARYThe LVN carries out nursing care plans for patients as per physician orders under the direction of a Registered Nurse. The LVN will perform daily activities with an attitude of humility. The LVN will approach each patient and coworker with respect and with the goal giving the highest quality patient care possible.
JOB DUTIES AND RESPONSIBILITIES1. Under the direction of the RN, perform skilled nurse visit that assists in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. This includes the prep time before the visit, visit documentation, and any follow up information that needs to be communicated to your supervisor. The documentation for this activity must be completed and in "submitted with signature" status within 24 hours of performing the visit. (EF)
2. Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care. (EF)
3. Makes after hours and weekend visits. (EF)
4. Participate in the PI program. (EF)
5. Provide care utilizing infection control measures that protect both staff and patient. (EF)
6. Assure the continuity of care through delivery of quality patient care. (EF)
7. Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations. (EF)
8. Demonstrate commitment, professional growth and competency. (EF)
9. Utilizing time efficiently to manage daily schedule. (EF)
10. Complies with all Home Health State and Federal Regulations as applicable (EF)
Hospital:
1. Adheres to organization-wide and other applicable policies and procedures.
2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations.
3. Performs within the prescribed limits of the hospital's Ethics, Compliance and Confidentiality Program guidelines.
4. Performs within professional Scope of Practice at all times.

Minimum Requirements
EDUCATIONAL REQUIREMENTSRequired: Graduate of an approved school of Vocational Nursing.
EXPERIENCERequired: 1 year hospital experience.
Preferred: Home health experience.
Special Skills and Abilities:Required: Computer skills, communication skills, ability to perform well in stressful situations
Preferred: Wound care skills, central line skills
LICENSURE / CERTIFICATION REQUIREMENTSRequired: Current License Vocational Nurse by the Texas Board of Nursing, Driver's License, and CPR certification required.
