Transitional Support Specialist - Mid Shift
Full-time, Pay Range: $18.28 - $21.78 - Various Locations
UCP Seguin of Greater Chicago is seeking compassionate, reliable, and dedicated individuals to join our team as a Transitional Support Specialist (TSS) - Mid Shift. This position offers 35-42 hours per week and provides an excellent opportunity to make a meaningful difference in the lives of adults with developmental and other disabilities. The Transitional Support Specialist works under the supervision of the Life Skills or Adult Services Administrator to provide direct support, personal care, skill development, and community-based services that promote independence, inclusion, and personal growth.
Position Qualifications
We are looking for candidates who are passionate about supporting others and committed to providing person-centered care. Qualified applicants should possess:
  • A valid Illinois Driver's License.
  • Proof of current automobile insurance.
  • A reliable personal vehicle for transportation responsibilities.
  • The ability and willingness to transport participants safely to appointments, community activities, employment sites, and other destinations.
  • The ability to obtain and maintain certifications in CPR/First Aid, Medication Administration, Defensive Driving, and Behavior Management Training.
  • Strong communication, organizational, and interpersonal skills.
  • A commitment to participant safety, dignity, confidentiality, and individual rights.
  • The ability to work independently while collaborating effectively with participants, families, guardians, healthcare providers, and team members.

Key Responsibilities
As a Transitional Support Specialist, you will assist participants with activities of daily living, including personal care, meal preparation, medication administration, housekeeping, mobility, and community engagement. You will support individuals in developing independent living, vocational, communication, recreational, and social skills while encouraging community inclusion and self-determination. Responsibilities also include implementing Individual Service Plans (ISPs), documenting participant progress, maintaining accurate records, participating in team meetings, and ensuring compliance with agency policies and safety procedures.
Schedule
Work schedules vary by program and location and may include weekday and weekend hours, as well as morning, afternoon, and evening shifts. Schedule assignments are based on participant needs and operational requirements and may change as needed.
