About Hope’s Destiny:

Hope’s Destiny is a leading outpatient Substance Use Disorder (SUD) and mental health treatment program located in Horsham, PA serving both adolescents and adults. Our mission is to provide comprehensive, compassionate, and evidence-based care to individuals struggling with substance abuse and co-occurring mental health disorders.

Job Description:

Hope’s Destiny is seeking a dedicated therapist to join our multidisciplinary team. The ideal candidate will have experience providing therapy to individuals with substance use disorders concerns in an outpatient setting, as well as a strong commitment to delivering high-quality, client-centered care.

This is a part-time role that will involve morning availability running a group (Monday through Thursday 3pm to 8pm).

Responsibilities:

- Develop and implement individualized treatment plans based on assessment findings, client goals, and evidence-based treatment modalities.

- Provide evidence-based therapy services, including individual, group, and family therapy, using modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Motivational Interviewing (MI), and trauma-informed care.

- Facilitate psychoeducational groups on topics related to substance abuse, relapse prevention, and coping skills.

- Collaborate with other members of the treatment team, including psychiatrists, nurse practitioners, and case managers, to coordinate care and ensure integrated treatment planning.

- Conduct crisis intervention and risk assessments as needed, providing appropriate referrals and resources for clients in crisis or experiencing acute symptoms.

- Maintain accurate and up-to-date clinical documentation, including progress notes, treatment plans, and discharge summaries, in compliance with state regulations and agency policies.

- Participate in multidisciplinary team meetings, case conferences, and clinical supervision sessions, contributing to case discussions, treatment planning, and program development initiatives.

- Stay informed about current trends, research, and best practices in the field of substance abuse and mental health treatment, participating in continuing education and professional development activities as required.

Education Requirements:

- A Master’s Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field which includes a practicum in a health or human service agency, preferably in a drug and alcohol setting.

OR

- A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 1 year of clinical experience in a health or human service agency, preferably in a drug and alcohol setting.

OR

- An Associate Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 2 years of clinical experience in a health or human service agency, preferably in a drug and alcohol setting.

Qualifications:

- Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or equivalent licensure in the state of Pennsylvania is preferred but not required.

- Minimum of 2 years of experience providing therapy to individuals with substance use disorders and co-occurring mental health disorders in an outpatient setting.

- Strong knowledge of evidence-based treatment modalities for substance use disorders, mental health disorders, and trauma-informed care.

- Excellent clinical skills, including assessment, diagnosis, treatment planning, and intervention techniques, with a focus on client-centered and strengths-based approaches.

- Strong interpersonal and communication skills, with the ability to establish rapport, build therapeutic alliances, and engage clients from diverse backgrounds.

- Proficiency in electronic health record (EHR) systems and documentation practices, ensuring compliance with regulatory and agency standards.

- Experience in working with higher levels of care (IOP, PHP).

Working Conditions:

  • This position primarily operates in a clinical office environment.
  • Occasional travel may be required to attend meetings, trainings, or conferences.
  • Flexible schedule may be necessary to accommodate evening or weekend sessions.
  • Ability to maintain confidentiality and professionalism in a fast-paced, client-centered setting.

Physical Demands: The employee is occasionally required to stand and be on their feet for long portions of time. The employee must occasionally lift and/or move objects up to 25 pounds.
