Job description

Case Manager Role and Responsibilities:

Case Manager will assist members with addressing issues that present as barriers to their care, accessing needed resources and services; fostering and maintaining family/significant other relationships and support. Support needed to maintain the member in the community, and acts as an advocate in identifying the member’s needs and taking active measures to address the member’s needs. These activities provide a description of case management but are not intended to be exclusive. Case Managers will monitor the members' care and report all cases of suspected abuse, neglect, or exploitation to law enforcement and regulatory authorities until resolution. Case Managers will be responsible to document the members files of all activities pertaining to members and their care and to perform face to face community visits as required.

Case Management includes engaging in and performing a range of activities and interventions to meet the member’s needs and advocating on behalf of the member to ensure that those needs are met while they reside in the community. 

EDWP(CCSP/SOURCE) Case Managers Qualifications:

  • Minimum education for Case Manager would consist of: BA or BS degree from an accredited college or university in the field of Social Work, Sociology, Psychology or related health care or human services discipline. 
  • Must have a minimum of two (2) years demonstrated and documented experience in the healthcare service industry or human services. 
  • Minimum skills and knowledge for a case manager would encompass the ability to effectively coordinate and communicate with clients, service providers, general public and other staff members.
  • Skill in establishing and sustaining interpersonal relationships, team building, group dynamics and problem solving skills and techniques.
  • Knowledge of human behaviors, gerontology, community organization, service system development along with social and health services intervention techniques and methodology.

Additional Training Requirements:

  • Case managers must demonstrate successful team participation, written and verbal communication skills, knowledge of local, state and regional resources and the ability to establish community networks, comprehensive service coordination, and resources mobilization.
  • Case managers must demonstrate a professional and personal commitment to advocacy, creativity, and ethics with an emphasis on promoting and implementing self-directed care.
