THE GROWTH YOU HELP PROVIDE AS A CASE MANAGER

This is a full-time position with a typical schedule that aligns with standard business hours. Flexibility may be required based on meeting attendance and client needs.

As a Case Manager, your day will be filled with impactful interactions and important responsibilities. You will regularly attend team meetings and Individual Service Plan (ISP) meetings to collaborate with your colleagues and ensure every client receives personalized support. Assisting in the admissions process will be a key part of your role, helping new individuals transition seamlessly into our services. You will also participate in internal investigations, which requires a keen attention to detail and a commitment to client advocacy. Each day, you will be engaged in meaningful work that aligns with the mission of our organization.

OUR IDEAL CASE MANAGER

  • Bachelor's degree in a related field of study
  • 2+ years of case management experience in the Human Service field

Knowledge of the Department of Developmental Disabilities and Day Habilitation regulations, as well as CARF standards, is preferred.
