Position Description

The Case Manager provides case management services, support, and referrals to appropriate services as needed to FO guests. The Case Manager works respectfully with populations experiencing homelessness and understands how to engage people who have likely experienced trauma and challenging life circumstances. The Case Manager completes assessments and accesses community resources and support services; the case manager must be flexible and have computer skills to navigate general use and learn the client database.

Essential Duties & Responsibilities:
  • Engage Franciscan Outreach guests and meet their needs via outreach, intake, assessment, goal setting, diversion, referral, and followup
  • Provide comprehensive case management services to all guests, including:
  • Intake and benefit assessments, goal setting, case plan development, progress monitoring, finance management, and assistance with housing applications
  • Develop, monitor, and complete service plans and goals with guests, motivate and encourage guests to complete service plans and goals
  • Mediate issues that arise between guests, families, and third parties, and conduct crisis management services
  • Provide life skills instruction in formal and informal settings, including, but not limited to, financial literacy, general life skill development, wellness education, and other groups and skill development as needed
  • Complete referrals for appropriate physical, educational, employment, mental health, and substance abuse issues
  • Utilize motivational interviewing and harm reduction strategies to support movement toward housing stability, increased skills and income, improved health, and achievement of personal goals
  • Assist in the development of linkage agreements with service providers
  • Become trained and proficient in the use of HMIS and Coordinated Entry System databases, and document all client encounters to agency standards
  • Develop collaborative relationships with other service providers, interns, and staff to coordinate care
  • Participate in group and/or individual supervision and staff meetings
  • Participating in training and staff development opportunities
  • Create a positive, welcoming, safe atmosphere for shelter guests, mainly by providing traumainformed services using a harm reduction philosophy
  • Various relevant tasks, such as research on community resources and clinical information, attending provider groups, accompanying guests to appointments, and any other related duties as assigned
  • 40hour work week

  • Skills needed:
  • Strong interest in and ability to work closely and respectfully with guests who have experienced homelessness and may be dealing with mental health symptoms, substance use disorders, trauma, physical disabilities, and health conditions.
  • Ability to demonstrate sensitivity to the needs of guests in crisis
  • Selfdirected and comfortable working independently, without constant direct supervision, and with a team
  • Ability to work from a Harm Reduction perspective and provide TraumaInformed Care
  • Experience with using Motivational Interviewing preferred
  • Ability to develop and maintain appropriate boundaries, including maintaining confidentiality
  • Ability to develop appropriate professional relationships with staff from a wide variety of races, religions, cultures, ages, educational backgrounds, sexual orientations, and identified genders
  • Able to work as a positive, supportive team member
  • Strong oral and written communication skills
  • Able to gather information from a variety of sources
  • Ability to use Windowsbased computer applications and client database
  • Ability to consistently maintain required documentation
  • Qualifications:
  • Bachelor's degree in social services or related field preferred
  • Experience working with people experiencing homelessness, mental health, and substance use disorders preferred
