Job description

The Position:

St. Patrick Center, a ministry of Catholic Charities of the Archdiocese of St. Louis, is seeking a Case Manager to be responsible for overseeing the daily care and support of clients at Rosati Place, specifically within the Semi-Independent (SEMI) program. This role provides individualized counseling, treatment coordination, rehabilitation support, advocacy, and comprehensive case management services to clients who have co-occurring mental health and substance use disorders. Case Managers work closely with clients to promote stability, independence, and overall well-being while ensuring access to appropriate resources and services. This position will work weekend schedules, requiring flexibility to meet the ongoing needs of the program and its clients.
