Job description
In support of the mission and mandate of the CMHA Health Centre, the Registered Nurse (RN) provides comprehensive primary care services in accordance with the Standards of Practice for Registered Nurses as set forth by the College of Nurses of Ontario (CNO). These duties are accomplished within an interdisciplinary team of professionals whose objective is to provide primary health care and follow-up for patients and families. The RN participates in clinical administrative functions, and participates in the development, implementation, monitoring and evaluation of CMHA Health Centre programs and services supporting individuals, families, and the community.
The RN is responsible for delivering quality nursing care to patients with a focus on the triage and comprehensive assessment of new and existing patients, providing patient-centered education and health teaching, disease prevention, as well as early intervention and supports for chronic health conditions, screening, vaccination programs, and the coordination of care. The RN contributes to quality improvement initiatives, supports organizational priorities related to health equity and access, and provides mentorship and clinical guidance to team members in collaboration with the Nurse Practitioner Lead. The incumbent will be required to work at the CMHA CHC and other locations based on operational needs.
RESPONSIBILITIES & DUTIES:
The Registered Nurse (RN) is responsible for providing comprehensive nursing care through therapeutic, preventative, supportive, and rehabilitative interventions to help patients achieve and maintain optimal health and functioning. As a key member of the interprofessional team, the RN will:
- Practice in accordance with the standards of the College of Nurses of Ontario (CNO), CMHA policies and procedures, medical directives, and applicable legislation.
- Conduct comprehensive patient intakes and ongoing nursing assessments, including physical, mental health, and psychosocial evaluations. Identify actual and potential health concerns, assess risk, determine urgency of care (triage), and implement appropriate nursing interventions, referrals, and follow-up.
- Provide direct nursing care, including health assessments, telephone triage and follow-up, medication reviews, wellness checks, health teaching and counselling, wound care, immunizations, point-of-care testing, blood pressure monitoring, ear irrigation, suture removal, and other nursing procedures as required and within scope of practice.
- Administer prescribed medications, including psychiatric medications, monitor therapeutic response and adverse effects, and report concerns to the most responsible provider (MRP).
- Respond effectively to acute mental health crisis using de-escalation techniques and trauma-informed, recovery-oriented, culturally safe, and patient-centred approaches to promote safety and positive outcomes.
- Coordinate patient care by arranging referrals, diagnostic investigations, appointments, follow-up care, communication of test results, and ongoing monitoring of outstanding clinical actions in collaboration with the interprofessional team.
- Collaborate effectively within the interprofessional team including, physicians, nurse practitioners, mental health professionals, and community partners to develop, implement, monitor, and evaluate individualized care plans, coordinated care, and population health initiatives.
- Promote health, disease prevention, and health equity through patient education, chronic disease management, provincial cancer screening programs, immunization initiatives, and other preventative health activities that improve health outcomes and reduce barriers to care.
- Lead the planning, coordination, implementation, and evaluation of vaccination and immunization clinics, ensuring appropriate vaccine inventory management, storage, documentation, and quality assurance in accordance with Public Health, Ontario Health, and organizational requirements.
- Maintain accurate, timely, and comprehensive clinical documentation within the electronic medical record (EMR), participate in chart reviews, and case conferences, and comply with CNO standards and organizational documentation requirements.
- Assist patients and families in identifying and accessing appropriate community resources to support health, recovery, and overall well-being.
- Participate in quality improvement activities and support the implementation of clinical policies, procedures, and chronic disease initiatives to promote safe, effective, person-centred care.
- Support the maintenance of clinic supplies, medical equipment, point-of-care testing quality assurance, and contribute to the efficient operation of clinical services.
- Provides support and training to employees, volunteers and students assigned to the program area and assists other agency employees where skills are applicable.
- Models professional accountability by fostering a collaborative and psychologically safe practice environment, supporting quality improvement initiatives, encouraging reflective practice, and promoting continuous learning.
- Demonstrate effective organization, time management, and critical thinking skills while working independently and collaboratively in a dynamic clinical environment.
- Support the mission, goals, and strategic priorities of the CMHA Health Centre and CMHA Windsor-Essex County by participating in team meetings, continuing education, mandatory training, and maintaining a safe, healthy workplace in accordance with organizational policies and occupational health and safety legislation.
- Perform other duties consistent with the scope and responsibilities of the position.
EDUCATION & SKILLS:
- Bachelor's degree in Nursing (BScN) is required. Registered member in good standing with the College of Nurses of Ontario (CNO) is required. Current Basic Life Support certification.
- Minimum two (2) years of clinical nursing experience relevant to the provision of primary care. Thorough knowledge and proficiency in current nursing physical assessments and treatment methods.
- Experience working with individuals experiencing mental health concerns, substance use concerns, homelessness, or other complex social and health needs is considered an asset.
- Advanced clinical assessment, triage, and care coordination skills. Advanced clinical knowledge in relevant mental health/ primary care specialty area, knowledge in the development and implementation of policies, procedures and educational programs; ability to analyze and interpret valid research findings in clinical practice; ability to work independently and collaboratively with members of the health care team; working knowledge of
- Knowledge of immunization standards and vaccine management required.
- Knowledge of mental health & relevant legislation, Ontario Health cancer screening programs, preferred.
- Computer literacy skills and working knowledge or various Microsoft Windows application is required.
- Bilingualism in both official languages at the advanced level is considered an asset.
- A valid Ontario Driver's License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.
Hours of Work: Thirty-Five (35) hour work week. The agency's regular hours of operation are Monday to Friday, 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.
Salary range: Grade 9, $37.84 - $44.81 per hour
Per 2022 - 2026 Collective Agreement
Applicable shift premiums are paid in accordance with the Collective Agreement, Article 17.11.
