Position Summary

Ideal Home Health Agency is seeking a compassionate, dependable, and organized Part-Time Registered Nurse (RN) to join our home health clinical team. The RN will complete admissions, recertifications, supervisory visits, and other required nursing visits while providing clinical oversight of Home Health Aides.

This position offers a flexible schedule and the opportunity to provide individualized care to patients in their homes while working closely with caregivers, physicians, and agency staff.

Key Responsibilities
  • Complete Start of Care/Admission assessments, recertifications, supervisory visits, and other required nursing visits.
  • Assess patients' medical, functional, psychosocial, and environmental needs.
  • Develop, review, and update individualized Plans of Care and ensure services are provided according to physician orders.
  • Conduct Home Health Aide supervisory visits, evaluate caregiver performance, and provide education or coaching as needed.
  • Monitor patient condition and promptly communicate significant changes to physicians and appropriate agency staff.
  • Provide patient and caregiver education regarding medications, safety, infection prevention, disease management, and the Plan of Care.
  • Complete accurate and timely clinical documentation in accordance with agency, payor, and regulatory requirements.
  • Coordinate care with physicians, caregivers, office staff, and other members of the interdisciplinary team.
  • Review caregiver compliance with the Plan of Care and address clinical or quality concerns.
  • Participate in chart reviews, quality improvement, compliance activities, meetings, and required trainings.
  • Maintain patient confidentiality and comply with HIPAA and applicable state and federal requirements.
Qualifications
  • Current, active, and unrestricted Pennsylvania Registered Nurse (RN) license.
  • Minimum one year of nursing experience required; home health experience preferred.
  • Strong assessment, clinical judgment, documentation, and communication skills.
  • Ability to work independently and manage a flexible patient schedule.
  • Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.
  • Current CPR certification.
  • Ability to meet required background checks, health screenings, and other pre-employment requirements.
