Advanced Practice Provider
(Nurse Practitioner or Physician Assistant
Location: High Desert region, Southern California
Compensation: $155,000-$170,000 annually
Schedule: Full-Time, On-Site
Hours: Monday-Friday, 8:00 a.m.-5:00 p.m.
Position Overview
We are recruiting on behalf of an established medical group for a full-time Advanced Practice Provider
for its primary care team in the High Desert region of Southern California. This single opening is open
to either a Nurse Practitioner or a Physician Assistant - the medical group welcomes candidates from
either background.
We are seeking a patient-centered Nurse Practitioner or Physician Assistant who is committed to
delivering high-quality, comprehensive primary care. Under the direction of the Medical Director, the
Advanced Practice Provider will manage an assigned patient panel, develop individualized treatment
plans, and coordinate care across clinical and support teams.
This opportunity is well suited for a collaborative clinician who values continuity of care, strong patient
relationships, quality outcomes, and multidisciplinary communication.
Key Responsibilities
• Perform comprehensive physical examinations and obtain detailed patient medical histories.
• Evaluate symptoms, establish diagnoses, and develop evidence-based treatment plans for
assigned primary care patients.
• Provide ongoing management of acute and chronic medical conditions.
• Order appropriate laboratory, imaging, and diagnostic testing.
• Review and interpret clinical results and adjust treatment plans as appropriate.
• Refer patients to specialists and other healthcare services to support continuity and coordination of
care.
• Collaborate with physicians, case managers, clinical staff, and other members of the care team.
• Maintain accurate, timely, and complete clinical documentation.
• Apply an understanding of Hierarchical Condition Category coding and managed-care
documentation requirements.
• Participate in clinical quality, performance improvement, and patient-safety initiatives.
• Educate patients and families regarding diagnoses, treatment options, preventive care,
medications, and lifestyle recommendations.
• Facilitate sensitive end-of-life, palliative care, and hospice discussions when clinically appropriate.
• Develop positive working relationships across clinical departments.
• Participate in provider meetings, staff meetings, and organizational initiatives.
• Support the medical group's mission, clinical standards, and commitment to excellent patient
service.
Required Qualifications
• Master of Science in Nursing, Doctor of Nursing Practice, or successful completion of an
ARC-PA-accredited Physician Assistant program.
• Active and unrestricted California Nurse Practitioner or Physician Assistant license.
• Active and unrestricted Drug Enforcement Administration registration.
• Minimum of two years of experience practicing as a Nurse Practitioner or Physician Assistant in
primary care, outpatient family medicine, or a closely related setting.
• Comprehensive knowledge of primary care, preventive health, chronic disease management, and
managed-care principles.
• Strong clinical judgment, critical-thinking skills, and the ability to prioritize patient care needs.
• Excellent interpersonal, communication, and active-listening skills.
• Ability to build strong relationships with patients, families, physicians, and clinic staff.
Preferred Qualifications
• Experience working within a medical group, managed care, Medicare Advantage, or value-based
care environment.
• Familiarity with HCC coding, risk adjustment, and quality-performance measures.
• Experience managing an established primary care patient panel.
Compensation and Benefits
• Annual salary of $155,000-$170,000, based on experience and qualifications.
