An Occupational Therapist is responsible for helping patients with physical, mental, or emotional disabilities to develop, recover, or maintain daily living and work skills.

PHC is a rapidly growing full-service Home Health Agency looking for an energetic and dedicated team member who is willing to learn new skills and improve our business processes. This is a fast-paced role in a growing company working in a very friendly and dynamic team.

We are growing quickly and looking for dedicated Home Health professionals to staff Cary/Raleigh/Durham amp; Surrounding areas providing excellent patient care. Accepting New Graduates!

Responsibilities
  • Evaluate patients' abilities and determine treatment plans (Start of Care) in consultation with physicians or by prescription to help patients improve or maintain their daily living skills
  • Assist patients with exercises and activities to improve their strength, coordination, and range of motion
  • Teach patients how to use adaptive equipment and assistive technology to perform daily tasks
  • Provide education and training to patients, families, and caregivers
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion, and function or teach compensation measures.
  • Coordinate care, including care management and discharge planning as needed with patient / responsible party, physician, other disciplines, and or external resources.
  • Collaborate with other team clinicians and healthcare professionals, such as physicians, nurses, and social workers, to provide comprehensive care to patients
  • Maintain accurate, objective and up-to-date patient records and documentation in accordance with regulations and agency policy/procedures
  • Stay current with new developments and techniques in occupational therapy through continuing education and professional development
Requirements
  • NC or Multi-State Occupational Therapy License
  • Current CPR Certificate
  • Preferred: 2-3 years experience working with patients with a variety of disabilities and conditions; prior home health experience
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Flexibility and adaptability to changing patient needs and treatment plans
  • Current Driver's License and reliable transportation
  • Great technology skills (use of iPad/Laptop to record visit notes, EMR)
  • Bi-Lingual (Russian/English; Spanish/English) is a plus
