Pay Range: $80k - $90k annually

Sequoia Home Health is growing and looking for experienced, compassionate Home Health LVN Clinical Coordinator to join our team!

Job Summary

We are seeking a Licensed Vocational Nurse (LVN) Clinical Coordinator to support home health intake, care coordination, and clinical operations. This role ensures timely admissions, regulatory compliance, and high-quality patient care.

Key Responsibilities

  • Manage patient intake, referrals, and admissions

  • Review documentation for home health eligibility

  • Coordinate care with physicians, patients, and families

  • Ensure compliance with Medicare, state, and Joint Commission regulations

  • Track hospitalizations, holds, and start/resumption of care (SOC/ROC)

  • Participate in IDT meetings and QAPI initiatives

  • Support field staff with clinical questions and urgent needs

Qualifications

  • Active LVN license

  • 3+ years nursing experience (some home health required)

  • Knowledge of home health, intake, or care coordination

  • Strong communication and organizational skills
