Job description

Pride Health is hiring a Registered Nurse to support our client's Home Health program in Shelbyville, KY (including Shelby County and parts of Eastern Jefferson County). This is a 3-month contract assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
Job Details:
Job Name: Home Health RN
Location & Coverage Area: Shelbyville, KY (including Shelby County and parts of Eastern Jefferson County)
Job Type: Local Contract
Duration: 13-week Contract (Possibilities of extension/ conversion)
Pay rate: $50 - $56/hr. on W2 (Negotiable - Pay is based on experience /educational qualifications)
Schedule: Monday to Friday from 8 am to 4:30 pm (32 hrs./ wk. guaranteed)
On-Call/ Weekend: Yes, one weekend a month.
Mileage: Reimbursement provided (from first patient to last patient, excluding commute from home)
Job Responsibilities:
  1. Perform Start of Care (SOC), recertification, resumption of care, and discharge visits.
  2. Conduct comprehensive patient assessments and develop individualized plans of care based on physician orders.
  3. Provide skilled nursing care, patient education, medication management, and disease management in the home setting.
  4. Monitor patient progress, evaluate treatment effectiveness, and update care plans as needed.
  5. Complete accurate and timely documentation using Homecare Homebase (HCHB) EMR.
  6. Complete and maintain OASIS documentation in compliance with Medicare and agency standards.
  7. Collaborate with physicians, therapists, case managers, and other interdisciplinary team members to coordinate patient care.
  8. Educate patients and caregivers on diagnoses, medications, treatments, and home safety.
  9. Ensure compliance with agency policies, regulatory requirements, and quality standards.

Qualifications:
  • Active, unrestricted Registered Nurse (RN) license.
  • Ability to work independently in a home health environment
  • 1 year of home health experience.
  • HCHB (Homecare Homebase) EMR experience.
  • OASIS Certification highly preferred.
