Midland Care Connection
Midland Care Connection is a not for profit, community based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
Hospice House
A peaceful, 12 bed, inpatient facility dedicated to providing compassionate, end of life care when a home setting is no longer feasible. Here, patients receive around the clock, skilled hospice care from a multidisciplinary team-nurses, social workers, chaplains, and other specialists-focused on managing pain and symptoms with dignity and respect. The environment is designed to feel like home, offering comfort not just to the patients but also to their loved ones, providing space for visitation, emotional support, and moments of peace in the final chapter of life.
RN Hospice House
Provides hands on, comfort focused nursing care to patients receiving end of life services in an inpatient hospice setting. The role centers on assessing and managing symptoms (especially pain), administering medications, and ensuring patients remain as comfortable and dignified as possible. The nurse also updates care plans, documents all care provided, and works closely with the interdisciplinary hospice team (physicians, aides, social workers, chaplains, and volunteers). In addition, they support and educate families, helping them understand what to expect and offering emotional reassurance during a difficult time. Overall, the position combines clinical skill with compassionate, holistic care focused on quality of life rather than curative treatment.

Full Time 3-12 hour shifts, 7:00pm-7:00am

Required
  • Active, unrestricted RN license (Kansas)
  • Graduate of an accredited nursing program (ADN or BSN)
  • Ability to assess patients and manage symptoms
  • Strong communication and documentation skills
  • Current CPR/BLS certification
  • Valid driver's license and reliable transportation

Preferred
  • Hospice or palliative care experience
  • At least 1 year of clinical nursing experience
  • BSN degree
  • Experience with electronic medical records (EMR)
  • Hospice/Palliative Care certification (CHPN)
