Job description

  Full-Time On-Call Role – Work schedule of 7 days on, 7 days off 5pm – 8am

  Preferred Experience – Prior hospice experience is preferred.

  Licensing – Must hold a valid Kansas or Compact Nursing License, current Licensed Practical Nurse (LPN) in good standing.

  Skills – Strong time management skills, organizational abilities and self-management, ability to work in a team environment, capacity to learn an electronic medical record system.

  Customer Service – Strong problem solving skills, proficient in phone, email, and written communication. Compassionate and effective communication skills working with patients and medical professionals on a 1-1 level.

  Transportation – Must willing to drive, have a valid driver’s license, reliable transportation, and auto insurance.

Responsibilities:

On Call LPN will work 7 days on 7 days off from 5pm – 8am taking calls to provide care and hospice services for a group of patients in the Topeka geographic area. Visits will take place where the patients reside; including patient’s private homes, Skilled Nursing Facilities, and Assisted Living Facilities. The On Call LPN will provide expertise regarding interventions for patients, inform physician of patient status changes, carry out physician orders, maintain accurate medical records, and communicate with Case Managers and patient’s loved ones.
