Licensed Vocational Nurse (LVN)
Lone Tree Post Acute
Starting Rate: $32/hour

Lone Tree Post Acute is currently seeking compassionate and dependable Licensed Vocational Nurses (LVNs) to join our nursing team on a full-time basis.

The primary purpose of this position is to provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants in accordance with current federal, state, and local standards and regulations, and as directed by the Director of Nursing Services or Nurse Supervisor.

Essential Duties & Responsibilities

  • Provide direct nursing care to residents in accordance with care plans and physician orders

  • Supervise and support Certified Nursing Assistants (CNAs) during assigned shifts

  • Monitor residents' conditions and report changes appropriately

  • Administer medications and treatments as prescribed

  • Ensure accurate and timely documentation of resident care

  • Maintain a safe, clean, and comfortable environment for residents

  • Follow infection control and safety procedures at all times

  • Communicate effectively with residents, families, physicians, and interdisciplinary team members

  • Assist in admissions, transfers, and discharges as needed

  • Respond promptly to resident needs and emergencies

Qualifications

  • Current California LVN license in good standing

  • Skilled nursing or post-acute experience preferred

  • Strong clinical and communication skills

  • Ability to work independently and as part of a team

  • Compassionate, dependable, and resident-focused
