Job description

Job Title: Family Nurse Practitioner (FNP)

Reports to: Family Medical Doctor / Medical Director

Employment Type: Part-time or Full-time (as assigned)

Job Summary

Delivers high-quality primary and preventive care for adults in an integrated community-mental-health setting, focusing on chronic-disease management, wellness promotion, and collaboration with behavioral-health teams.

Key Responsibilities
  • Conduct full health assessments, diagnose & manage acute/chronic conditions.
  • Provide medication management, patient & family education, and minor procedures.
  • Order/interpret diagnostic testing.
  • Collaborate with physicians, therapists, case managers; mentor nursing staff.
  • Maintain thorough EHR documentation and participate in qualityimprovement initiatives.
  • Qualifications
  • MSN or DNP, national NP board certification (AANP/ANCC).
  • Active IL NP license; Controlled Substance/DEA & FullPractice Authority preferred.
  • ≥12 yrs primarycare NP experience; behavioralhealth exposure valued.
  • Proficiency with EHR (athenaOne) and strong teamwork/communication skills.
  • Work Environment & Physical Requirements
  • Ambulatory clinic environment with some standing/walking; must meet standard clinical credentialing and vaccination requirements.
