You will be an integral member of a multidisciplinary team that includes Nurse Practitioners and Clinical Exercise Physiologists, all dedicated to delivering excellence in patient care. In your role as a Licensed Clinical Social Worker, you will:

• Conduct comprehensive assessments to evaluate patient behavioral health and identify their unique needs.
• Develop and manage individualized treatment plans grounded in evidence-based practices, ensuring alignment with current literature and best practices.
• Provide virtual psychotherapy sessions, lasting 45-50 minutes each, tailored to facilitate patients’ personal growth and behavioral changes.
• Collaborate closely with an integrated clinical team to design and launch our innovative virtual-first cardiac recovery program, enhancing the overall patient experience.
• Monitor and re-evaluate treatment plans regularly, adapting approaches as necessary to ensure both safety and effectiveness for each patient.
• Implement behavioral change techniques such as mindfulness and motivational interviewing to empower patients in their recovery journey.
• Document patient interactions accurately and within required timeframes, facilitating seamless communication with the clinical team and partner health systems.
• Promote engaging patient interactions, working diligently toward achieving both individual and programmatic goals.
• Make informed decisions to modify treatment plans according to professional standards and regulatory requirements, ensuring optimal patient care.

You will primarily serve a diverse population of patients facing various behavioral health challenges, including those in need of mental health support and individuals participating in our cardiac recovery program. Your work will directly impact those seeking holistic health solutions, reinforcing the importance of integrating mental and physical health care.

The ideal candidate will possess:

• A valid New York State license as a Clinical Social Worker (LCSW).
• A Bachelor’s degree in Social Work or a related field; advanced degrees are a plus.

• Strong interpersonal and communication skills, enabling effective collaboration with an interdisciplinary team.
• Experience in delivering teletherapy services or working within remote clinical teams will be highly regarded.
