Primary Care Physician – New Haven, CT

Compensation:
Base Salary: $215,000–$300,000(commensurate with experience and location)
Incentive Bonus: Quality-based performance bonuses

Position Overview

We are seeking a dedicated Primary Care Physician to join our growing team in New Haven, CT. Our innovative network of neighborhood primary care centers is designed to provide equitable, patient-centered, and value-based healthcare to local Medicare populations.

Our physicians are supported by multidisciplinary care teams and advanced clinical technology, allowing you to focus on what matters most—delivering quality outcomes and preventive care, not patient volume.

Key Responsibilities
  • Deliver comprehensive, patient-focused primary care services within a value-based care model.

  • Manage a patient panel of approximately 13–16 patients per day.

  • Conduct visits averaging 20 minutes for regular appointments and 40 minutes for new or complex cases.

  • Collaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker.

  • Utilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations.

  • Maintain accurate documentation using Canopy and Greenway EMR systems.

  • Participate in community health initiatives such as wellness classes, exercise groups, and social engagement activities.

Schedule & Environment
  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM

  • Clinic Model: 3–5 providers per site (mix of MDs and NPs)

  • Transportation Support: Patient pickup service within 5 miles of the clinic

  • Community Engagement: Regular on-site patient wellness events (yoga, dance, spa days, and more)

Qualifications
  • Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)

  • Board Certified in Internal Medicine or Family Medicine

  • Active, unrestricted State Medical License and DEA

  • Minimum of 1 year post-residency clinical experience (no residents)

  • Bilingual (Spanish) preferred

  • Geriatrics Fellowship training, preferred

Compensation & Benefits
  • Base Salary: $215,000–$300,000+ Incentive Bonuses
